Additional Information
- Account Number: Your account number can be found in the upper right area of your invoice.
- After Hours Fee: All office visits after 5:00pm Monday – Friday and all weekend and holiday visits include an office visit fee plus a $35 After Hours fee that some insurance plans cover. If your insurance plan does not cover the After Hours fee, you will receive a billing statement for your review and payment.
- FMLA Paperwork: A $25 fee will be charged prior to the completion of FMLA forms or other paperwork not directly related to medical insurance reimbursement of charges incurred at ARC. Please allow five business days for the completion of these forms.
